Abdominal Transplant Fellowship Program

Application Instructions

Electronic submission of materials is preferred. Please attach the required materials below to an email and send to: txpfel@u.washington.edu

  • Abdominal Transplant Fellowship Program Application. (Download the PDF Application Form, save to your local machine, fill out, and then return to the email above.)
  • CV
  • A Personal Statement (maximum of two pages) that:
    • Describes your professional objectives and interest in transplantation, including relevant experiences during training that impacted your decision to go into transplantation, and
    • Includes the following attestation:

“I certify that the information contained in my application to the University of Washington Abdominal Transplant Fellowship Program is correct to the best of my knowledge. I authorize investigation of all matters contained in my application and agree that any misleading or false statements would be cause for rejection of this application or would be sufficient cause for dismissal after my appointment. I hereby authorize my present and past employers and programs to furnish UWMC with their records of my service. “

  • Graduates of foreign (non-U.S./Canadian) medical schools must, in addition, provide proof of:
    • ECFMG certification
    • Successful completion of USMLE Steps 1, 2 (CK & CS), and 3

All applicants must also arrange for three letters of recommendation to be sent directly to the Director of the Fellowship Training Program, including one from the Chair or Director of your residency program and two others from faculty or professional staff of your program or hospitals in which you have worked. Please have these letters mailed to:
