Virginia Mason

To ensure that your hospital privileges are ready on Day 1, you must download and complete the following 3 forms. Send them to Susie Rheinberger at the address listed below*. Failure to do so in a timely manner will prohibit participation in the Surgery Clerkship at this site. Forms are also available on Virginia Mason's GME website.

2. Disclosure statement


3. Fitness for Duty: To be completed and signed by an Authorized Representative at UW Hall Health. If you cannot get to Hall Health, please fax the Authorization Form from UW Medicine along with your Fitness for Duty form to Hall Health, ATTN: Health Sciences Program at 206.616.4683. Please indicate that you would like both forms to be faxed to Susie Rheinberger.

A flu shot is required during flu season for participation at Virginia Mason. Shots may be requested at UW Hall Health Immunization Clinic. Please contact the clinic as soon as possible to schedule a time (myshots@u.washington.edu or 206.685.1011).
