A 52 year old man presents with a history of mild trauma

DISCUSSION QUESTIONS:
9.) What antibiotics should be given?

Broad spectrum antibiotic coverage is needed to cover gram positives, anaerobes, and enteral bacteria. This could be Ampicillin-Sulbactam or Imipenem-Cilastatin or a combination of drugs as would be used in an intraabdominal infection. After identification of organisms, the antibiotic therapy may be directed towards these organisms. High dose penicillin is good if clostridial infection is suspected. The addition of clindamycin to high dose penicillin is helpful because clindamycin will block the bacterial toxin produced by Clostridia.
