To increase the resident’s appreciation of the impact of chronic disease upon their patients suffering from the same. 2. To assist residents to define a facilitative role in relationship to chronically-ill patients. Objectives
The resident will understand the impact of chronic illness upon patients and their families. 2. The resident will develop the appropriate attitudes necessary for managing patients who will not get well. 3. The resident will know how to make and sustain a facilitative role with chronically ill patients. 4. The resident will gather an intimate knowledge of the experience of one of his or her patients regarding their experience of their chronic illness. Methods

The resident will recruit a patient from their practice who is suffering a chronic disease to act as a tutor for the resident throughout the year regarding what it is like to live with chronic disease. Resident and patient will agree to discuss a number oftopics throughout the year and a home visit will be made sometime in the year.
The resident will keep a journal and reflect through the year on the lessons learned in caring for this patient. At year’s end, these lessons will be summarized in conference with the program behavioral scientist. Evaluation
The patient will provide a written evaluation of the resident’s performance.
- The resident will debrief the learning experience with the program behavioral scientist.
- The resident will complete the electronic post-test for the module found on the curriculum website.
- The resident will complete the electronic evaluation for the learning module found on the curriculum website.
