Getting Started

UW LEAH Trainees must complete the registration requirements for the UW LEAH Program.

Section 1

• UW LEAH Orientation
  Attend and participate in the UW LEAH Orientation.
• Trainee Registration
  Complete the UW LEAH NIRS Trainee Registration Form and e-mail to Katie Albertson katie.albertson@seattlechildrens.org.  
• Trainee Profile
  Complete the UW LEAH Trainee Profile Worksheet and email to Katie Albertson katie.albertson@seattlechildrens.org.
• Release of Information
  Complete UW LEAH Release of Information Form and email to Katie Albertson katie.albertson@seattlechildrens.org.​
• UW NetID and Email​
  Create a UW NetID and open an email account if you do not already have one. You will need a UW NetID to connect to UW services, access your ePortfolio, and other electronic activities during your UW LEAH training. If you are not eligible to create a UW NetID, please contact Katie Albertson. katie.albertson@seattlechildrens.org.  ​
• Seattle Children’s ID Badge
  All trainees must wear a Seattle Children's hospital ID badge or other approved nametag while engaged in UW LEAH  clinic activities.
  Email Christine Clare requesting a badge and she will initiate the onboarding process.
• UWMC Professional Image Policy
  Read the UWMC Professional Image Policy

Finalize the Registration

Gather all forms as requested above. Return all appropriate documents to:
