Cellular Phones


Ordering Information

Please read the following information in its entirety and call 598-2300 if you have any further questions.

To dispose of your old cell phones:  http://www.washington.edu/cac/care/phone/cellular.disposal.html.

  1. Use the Cellular Phone Authorization form to gather all the information that you will need to order a new or replacement phone. The form must be signed by the director/manager of the department and the appropriate associate administrator. Keep a copy of this form with the PO for future reference.

  1. Go to http://www.washington.edu/itconnect/phones/cellular.html to find up-to-date pricing information from Cingular Wireless, Nextel, T-Mobile and Verizon Wireless. Use this information to choose a cellular phone and type of monthly service. You may contact the account manager for each vendor regarding compatible accessories and prices for your selected model.

  1. Submit a standard purchase order requisition to the Purchasing Department for the phone you would like to acquire.Please do not include an encumbrance for air time. See purchase order example found here. ** Confirming purchase orders for telecommunications equipment are not allowed.**

If you are requesting a replacement phone and it is being offered for free by the vendor a purchase order is not needed. Simply send an email request to ccbf-cellular@cac.washington.edu indicating the cellular phone number and type of phone needed. Forward the Cellular Phone Authorization form to the Cellular Help Desk, box 354840.

  1. Fax or campus mail a copy of the purchase order and the cellular phone authorization form to:

Delivery is usually about 7-10 business days from submitting the order. The cellular phone will have a number assigned, and will be usable after you receive it and charge the battery.

The cellular carrier (Cingular, Nextel, T-Mobile or Verizon) will send monthly invoices for the recurring service and airtime to Computing & Communications Business & Finance, which will pay it from the budget you specified on the authorization form. A copy of the detail invoice will be sent to the contact person listed on the requisition unless other arrangements are made.


Billing Information Changes

Billing Changes: Send an email request to ccbf-cellular@cac.washington.edu.Please include the following information:

  • cellular number
  • current budget number and budget name
  • new budget number and budget name

User Name Change: Send an email request to ccbf-cellular@cac.washington.edu. Please include the following information:

  • cellular number
  • old users name
  • and new users name, phone, box number and email
