Transitions of Care rotation website

Transitions in care are times of vulnerability for patients and providers, with change both in physical location (i.e., hospital to home, skilled nursing facility or temporary housing situation) and in care team (i.e., hospitalist to PCP). This elective provides the resident an opportunity to evaluate patients in transition in multiple settings, thus informing future management of transitions in his or her own practice. The resident will visit and develop familiarity with common post-discharge settings (long-term care, visiting nurse services and shelter/respite) and provide post-discharge care in a variety of settings. He or she will be asked to discuss the impact of transition with patients directly and reflect on selected cases in a structured manner. A syllabus of selected literature and other relevant materials will be provided and discussed with coordinating faculty.
