Application Instructions

1. Contact your Student Affairs office for processing procedures for ERAS applications.
2. Submit the following items through ERAS:

• Completed ERAS application
• Personal statement
• Medical School Transcript
• Three Letters of Recommendation
• USMLE Transcript
• Medical School Dean's Letter

3. In addition to your ERAS application, the UW requires three supplemental recommendation forms to complete your application. Please submit the supplemental forms to the address below or to amweiss@u.washington.edu or FAX: 206-598-9979 (Attn: Angela). Do NOT submit them on ERAS. Click here to download the forms.

4. After the screening process has been completed, all applicants will be notified immediately by e-mail of their interview status.
