PEDS 652 – Pediatric Clinical Clerkship – Idaho Falls, ID

Idaho Falls is the perfect location for people looking for a diverse opportunity to discover the wonders of Yellowstone, Grand Teton National Parks, the Craters of the Moon National Monument, or Henry’s Fork of the Snake River. It is a great location for the outdoor enthusiasts to enjoy some of the best skiing, camping, fishing, and hiking that there is to offer.

Come and enjoy the greenbelt along the falls in Idaho Falls, ‘the best little zoo in the west’, opera, symphony, or drama there are diverse events to please everyone. It is also the location of the Idaho National Engineering and Environmental Laboratory with some of the nation’s most difficult technical problems are worked on by engineers, scientists, chemists, and physicists. It is the home of the largest concentration of technical professionals in the northern Rockies.

Credentialing

The Idaho regional office will reach out 6-8 weeks ahead of your rotation. Please respond to all requests as soon as possible. For any questions, please contact Sarah Keshian.

The sites require current TB testing, please comply with each site’s requirements.

Eastern Idaho Regional Medical Center

Student credentialing and onboarding for clerkships rotating through Eastern Idaho Regional Medical Center are now handled via ClinicianNexus, a web-based platform. The Regional Office Administrator will add the rotation dates to ClinicianNexus and following, the Eastern Idaho Regional Medical Center team will approve the rotation and review any onboarding documents.

All scheduling will continue be completed through the UWSOM’s registrar’s office, do not make any changes or requests through ClinicianNexus.

Housing

For this required clinical clerkship students will be contacted by Natalee Snarr from the Pediatric Center. A 2-bedroom apartment located within two miles of the office and hospital, is close to main shopping and eating areas of Idaho Falls. The apartment is family friendly, and we welcome families. Pets are NOT allowed. Apartment includes a garage and washer/dryer.

Transportation

It is best to bring a car as public transportation is limited. Winter driving conditions are likely October to March.

Orientation

On morning of orientation, bring your student ID, current driver’s license, and the signed confidentiality agreement located within the orientation booklet. Students will report to The Pediatric Center at 8:00AM and you will be introduced to your preceptors/administrators. Students will need their own oto-opthalmoscope and stethoscope. White coats and ties (males) are not necessary, but name tags are required. You will begin to see patients that afternoon.

Experience

The rotation will be divided into a three-week inpatient and a three-week outpatient portion.

3-Week Hospital Inpatient

For three-weeks, the student will be full-time participating in Pediatric Inpatient care at EIRMC.  The expectation is for the student to participate in all levels of care, from assessment at admission, daily management care and discharge of the patient. The day will begin at 6:00am at the hospital to pre-round on patients. It is expected that they will create a medical student note in the EMR (Meditech). Students will need to be prepared for multidisciplinary rounds at 9:00am, followed by family-centered rounds. During rounds, the student will do oral presentations – a full H&P on any newly admitted patients and SOAP note presentation for all others. Students will need to ask the attending their preferred style for presentations and adjust accordingly. 

After rounds, any pending labs or imaging will need to be followed up on and results shared with the team. Students will finish their day at 6:00pm. Students will need to be available to help the attending or residents with any needed tasks. Students should ask for specific feedback often to improve their skills.  The student can work on Aquifer cases/NBME study questions/assignments during any down time. Monday through Thursday night, the student will need to be available by phone for any new admissions.  Friday-Sunday night, the student will not be on call. On the weekends, there are no formal rounds. If they have a patient that is staying through the weekend, they should do individual rounds on them – for the student’s learning/continuity only. Students will not have any new assigned patients over the weekend. Every Friday the student will present a 5 – 10 min topic of their choice from something they are learning on a patient or in their studies.  

3-Week Outpatient Clinic

Each morning newborn rounds will take place at EIRMC and Mountain View. Start time and location will vary based on volume and attending, but no later than 10:00am. If there are no newborns, the student will go to The Pediatric Center from 8:00am – 5:00pm and work with rotating attendings. Once per week during the three-week rotation, the student will work from 12:00pm – 8:00pm. There are no weekend requirements during this portion of the rotation.

For clinic visits, if parents agree, the student will go in first to do a focused history and exam. The student will take notes enough to present the patient to the attending, after which the attending will go into the patient room to verify history and repeat exam and discuss assessment and plan with the family. If things are getting busy in the schedule, some attendings may have the student just do history while the attending does the exam.  If behind, the student may need to simply shadow. Every effort will be made so that the shadowing is to a minimum to allow constant student involvement and engagement.
