Here\u2019s a step-by-step that I hope will help:<\/p>\n<ol>\n<li><strong>Credits<\/strong>: Usually this is &#8220;4&#8221; or &#8220;8\u201d. Credits are given on a basis of &#8220;2&#8221; credits per week of full time clinical experience. Feel free to leave it blank we can fill it in. BTW \u2013 any time I say \u201cleave it blank\u201d and the system seems to insist that you write something, feel free to put in \u201cXX\u201d or some asterisks or \u201cI have no idea\u201d and we can still fill it in.<\/li>\n<li><strong>Clerkship director\/faculty<\/strong>: Feel free to put in your name here.<\/li>\n<li><strong>Student number<\/strong>: That\u2019s the UWSOM student ID. If you don\u2019t have it leave it blank \u2013 we have them all.<\/li>\n<li><strong>Final grade<\/strong>: All students in an &#8220;Elective&#8221; course (not an APC or APC Sub-I course) are limited to\u00a0a &#8220;Pass&#8221; or &#8220;Fail&#8221; for a final grade.<\/li>\n<li><strong>12 domains marked as Likert scores<\/strong>: Each one of these has some verbiage that is supposed to make it clear what we mean by levels of performance for \u201cKnowledge of Subject\u201d, \u201cData-Gathering Skills\u201d etc. Mark for each or \u201cN\/A\u201d if it\u2019s something the student really doesn\u2019t do.<\/li>\n<li><strong>Bioethical Component and Mini-CEX<\/strong>: You can just type in \u201cN\/A\u201d.<\/li>\n<li><strong>Final Examination Grade<\/strong>: You can just type in \u201cN\/A\u201d.<\/li>\n<li><strong>Overall Assessment<\/strong>: Almost everyone is \u201cMeets or Exceeds Expectations\u201d. If the student is \u201cMeets or Exceeds Expectations\u201d, leave the next text box blank. If you click \u201cNeeds Development\u201d or \u201cUnacceptable\u201d, explain it in the text box that comes next.<\/li>\n<li>C<strong>oncerns about Clinical Performance or Concerns about Professional Behavior\/Conduct<\/strong>: If marked \u201cYes\u201d, it needs explanation in the box below. If \u201cNo\u201d, definitely leave the text box blank (typing into the boxes even the words \u201cNo concerns\u201d flags the system that there is a problem, so better to leave blank).<\/li>\n<li><strong>Areas of strength\/areas for work-growth<\/strong>: here, we ask for 2-4 bullet-point-type comments. If there was a problem that didn\u2019t really rise to the level of the \u201cConcerns\u201d boxes above, you can put it in here using some constructive feedback wording.<\/li>\n<li><strong>Summary of overall performance (the \u201cMSPE\u201d Box)<\/strong>: Here, we type in the overall commentary, trying to touch on all the Competencies. The Dean\u2019s office has said this must be &lt;300 words \u2013 not so easy!
