Thom is a 36 year-old Caucasian male with Psychosis. He has severe and chronic delusions that revolve around religious and somatic themes. Thom has been incarcerated many times with various assault charges, the most recent being life-threatening to the victim. His mother has been very supportive throughout the years and has been a strong advocate for Thom. He was released from jail to a community placement two weeks ago. His psychiatrist says that his mediations are stable. Thom does not like the facility he is residing in. He is maintaining, but in becoming increasingly agitated. His case manager is visiting him daily and researching other housing options. Thom said he is "coping okay" but wants to move out as soon as possible. He is not capable of working at this point in time.
