Collaboration: Using the WFI-EZ
In 2011-2012, a pilot version of the WFI-EZ was developed. We asked the National Wraparound Initiative community for volunteer sites to pilot test the WFI-EZ. Many sites volunteered to be part of the pilot test. We are tremendously grateful for their participation! For a full description of our initial findings, click here.
The WFI-EZ may only be used by approved collaborators of the Wraparound Evaluation and Research Team. In addition, collaborators must agree to meet the qualifications for use (see below).

Qualifications for Use
The WFI-EZ is a brief, self-administered version of the WFI-4. Instead of conducting lengthy interviews, the respondents complete a four page survey on their own, either on paper or online. Some sites may choose to read the questions out loud to respondents.

Fees For Use
In order to offset the costs of conducting reliability and validity studies; creating training materials; and managing the process of supporting collaborators, our Research Team must charge a fee for use of the WFI-EZ. We are asking new collaborator sites to agree to an initial one-time $1200 user's start-up fee, as well as an annual $1000 license fee. Collaborators with multiple sites that will want individual data reports and training toolkits will be asked to pay $350-500 annually for each additional sub-site (depending on size) or program after the first $1000 fee. Collaborators that renew their license annually will automatically receive new or revised WFI-EZ support materials and unlimited access to the Online Data Entry and Reporting System (WrapTrack).
A note for collaborators! Student collaborators or small programs for whom these fees represent a burden can request a reduction or waiver of fees. Just email us to discuss further.

STEP TWO: After submitting this form, you will receive an invoice from C4C for the appropriate amount due to the University of Washington.
STEP THREE: After we have received payment, you will receive information from our research team on getting access to the WFI-EZ materials. If your site needs materials before payment can be made, please email us at email@example.com.
