The WWAMI Regional Office will ask the student to complete the following WRITE Preceptor Performance Evaluation by Student mid-way through (first part of April) and at the end of the rotation (mid-June) for each WRITE preceptor with whom they have had significant contact (see next page). The forms are to be completed and submitted to their WWAMI Regional Office. After the WRITE Site’s primary preceptor has submitted the student’s grades, a summary of this information will be shared with the individual WRITE Site preceptors and will be used by the WRITE Steering Committee for overall site evaluation.
