Instructions for completing the reporting survey
You will be using this survey three times to report the activities and impacts associated with your CTOP grant. You must submit this survey by the following dates to receive reimbursement for your eligible expenses:
- October 31, 2008
- February 28, 2009
- June 15, 2009--Please note date change

What you will need to complete the survey
You may find it helpful to have the following information handy before starting the survey:
- To log in to the survey, you will need the organization email address you provided in your CTOP grant proposal.
- The impact areas identified in your grant proposal.
- Records of your activities, inputs, outputs, and impacts during the reporting period.
