uAllergies
uAdverse reactions or allergies to any medications used in the past
uA list of health care providers or specialists the patient is currently seeing or has seen in the past year
uHelpful: the patient’s approximate level of understanding and communication accommodations that are needed.
It is helpful to include the patient’s approximate level of understanding so the health care provider can direct as much information as possible to the patient.

Please mention any accommodations that might be need to enhance communication.  If picture books are helpful, please plan to bring the ones that that will be easily recognized by the patient.
