background_repeat=&#8221;repeat&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<h1 style=\"text-align: center;\">Immune Tolerance and Inflammation in Lung Transplant Rejection<\/h1>\n<p><strong>P.I.:<\/strong>&nbsp;<a href=\"https:\/\/pulmccsm.uw.edu\/faculty\/eric-morrell-md-ma\" target=\"blank\" rel=\"noopener\">Eric Morrell, MD, MA<\/a><br \/>\nActing Instructor, Pulmonary<\/p>\n<p>Background: Chronic lung allograft dysfunction (CLAD) is the most common cause of death in lung transplant (LTx) recipients. Infection and acute cellular rejection (ACR) are two of the most important risk factors associated with the development of CLAD, however the mechanisms that inform these epidemiologic observations are not understood. The PD-L1\/PD-1 pathway plays a key immunologic role in acute lung injury. We hypothesize that activation of the PD-L1\/PD-1 pathway is distinct in subjects who have infection vs. ACR, and that regulation of this pathway during these acute conditions contributes to the development of CLAD.<\/p>\n<p>Aims: 1) Determine whether PD-L1\/PD-1 expression and intracellular cytokine staining on blood\/alveolar leukocytes is distinct in subjects with ACR vs. infection; 2) Determine whether PD-L1\/PD-1-mediated plasma cytokine\/chemokine concentrations are distinct in subjects with ACR vs. infection; and 3) Comprehensively characterize alveolar leukocyte subtypes using single-cell RNA-sequencing (scRNAseq).<\/p>\n<p>Approach: Aim 1: Test whether PD-L1\/PD-1 pathway activation is distinct in subjects with ACR vs. infection by using cytometry time-of-flight to prospectively characterize PD-L1\/PD-1 expression and intracellular cytokine staining on blood\/alveolar leukocytes from subjects referred for bronchoscopy.<\/p>\n<p>Aim 2: Test whether cytokines\/chemokines regulated by the PD-L1\/PD-1 pathway are distinct in subjects with ACR vs. infection.<\/p>\n<p>Aim 3: Identify novel leukocyte subtypes that are present in subjects with ACR vs. infection by applying scRNAseq to alveolar leukocytes from a subset subjects from Aim 1.<\/p>\n<p>Significance: These novel human translational studies will inform future mechanistic in vitro and animal studies that seek to modulate the PD-L1\/PD-1 pathway in experimental models of CLAD.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][\/et_pb_section]<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Immune Tolerance and Inflammation in Lung Transplant Rejection P.I.:&nbsp;Eric Morrell, MD, MA Acting Instructor, Pulmonary Background: Chronic lung allograft dysfunction (CLAD) is the most common cause of death in lung transplant (LTx) recipients. One of these is abnormal glucose metabolism, where CF-related diabetes (CFRD) is common. More recently, another abnormality of glucose metabolism has been recognized; namely late hypoglycemia following oral glucose loading. In this study, we propose to test the hypothesis that the post-glucose load hypoglycemia observed in patients with CF results from a deficient counterregulatory hormone response and\/or an insulin response that is exaggerated and delayed. This increased insulin response could be the result of an exaggerated incretin hormone response or altered gastric emptying. To address this hypothesis, we will perform a 3-hour oral glucose tolerance test during which we will measure counterregulatory, islet and incretin hormone responses and determine the rate of gastric emptying using acetaminophen. To determine whether patients with CF and late hypoglycemia also have episodes of hypoglycemia during daily living that includes mixed meals, we will use a continuous glucose monitoring system (CGMS) to examine 24-hour glucose profiles for 3 days. All these measures will be compared between patients with CF who develop late hypoglycemia, CF patients who do not develop hypoglycemia, and age and body mass index-matched healthy controls. The findings from this study will provide important new information regarding the mechanism(s) responsible for the late hypoglycemia observed in patients with CF and the data could be used as the basis for future grant applications.
