It is imperative to keep your medical license up-to-date in order to continue providing patient care. A renewal form will be mailed to you directly by the Washington State Department of Health. Please note “It is the responsibility of the licensee to renew their license, whether the notice has been received or not (WAC 246-12-030(4)).”
You are responsible for sending the appropriate fees and any required documentation directly to the Washington State Department of Health. Please do not delay in sending these in and allow sufficient time for your renewal to be processed prior to the expiration date.

How to request reimbursement of your medical license renewal fee
You may be eligible for reimbursement of your renewal fee by GIM (processed through UW Physicians). Reimbursement is made in accordance with departmental policies. You must have an appointment with GIM of at least 50% FTE and provide clinical care at a departmentally approved clinical site (HMC or UWMC) in order to be eligible for reimbursement. To request for reimbursement of your renewal fee:
1) Download and complete the Medical License Reimbursement Form
2) Provide a copy of your renewed license (once you receive it)
3) Send the completed form and copy of your renewed medical license to:
UW Physicians Payroll
