Patient Rights & Responsibilities

As a patient of Hall Health Center, you have rights and responsibilities.


  • Access to care, treatment, or accommodations
  • To an interpreter, if you do not speak or understand English
  • Humane care and treatment
  • Accurate, current, and complete information
  • No care can be provided to you without your consent
  • Be informed of any research aspect of your care and to refuse to participate
  • A second opinion regarding diagnosis or treatment
  • Know who is counseling, caring for, or treating you
  • Information regarding the scope and availability of services
  • Information regarding fees and payment policies for service
  • Voice your concerns or complaints about your health care
  • Have information regarding advance directives
  • Have your health care information protected to ensure privacy and confidentiality
  • Access to your medical records through eCare or by completing and submitting a Release of Information form


  • Provide complete and accurate health information to those caring for you
  • Participate in discussions and ask questions about your care
  • Satisfy out-of-pocket costs associated with your care or make payment arrangements in a timely manner
  • Not give medications prescribed for you to others
  • Show respect to Hall Health staff and patients
  • Not lend your Husky Card or insurance card to others
  • Keep your appointments OR notify Hall Health at least 24 hours in advance if you need to cancel or reschedule
  • Watch your children to prevent safety issues and avoid disturbing other patients and families
