Patient Navigators work with patients, their families, and caregivers to overcome screening barriers such as lack of transportation, limited English proficiency, lack of childcare, low health literacy, or a general mistrust of the health care system. Patient navigation services may be provided by Community Health Workers (CHWs), nurses, social workers, or patient navigators. Patient navigation services do any of the following:&nbsp;&nbsp;&nbsp;<\/p>\n<ul class=\"wp-block-list\">\n<li>Provide <strong>patient reminders.&nbsp;<\/strong><\/li>\n<li><strong>Reduce out-of-pocket costs.<\/strong>&nbsp;<\/li>\n<li><strong>Reduce structural barriers<\/strong> such as transportation assistance, using interpretation services (see section on <strong><a href=\"https:\/\/depts.washington.edu\/hprc\/research-impact\/thematic-networks\/arc-nw\/ebi-compendium\/ebis-and-how-partners-use-them\/reducing-structural-barriers\/\">EBIs: Reducing Structural barriers<\/a><\/strong>).&nbsp;<\/li>\n<li><strong>Provide group or one-on-one education<\/strong> to patients on cancer and cancer screening.&nbsp;<\/li>\n<\/ul>\n<div class=\"box-outer\">\n<div class=\"box one box-left box-tan\">\n<div class=\"tile\">\n<h3 class=\"has-text-color\" id=\"ctw-reach\" style=\"color:#485c7e\">Turning Strategies into Action<\/h3>\n<\/p>\n<div class=\"wp-block-image is-style-default\">\n<figure class=\"alignright size-half\"><img decoding=\"async\" src=\"https:\/\/depts.washington.edu\/hprc\/wp-content\/uploads\/2024\/09\/Peninsula.png\"><\/figure>\n<\/div>\n<p><strong>Peninsula Community Health Services (PCHS)<\/strong> offers a wide range of health care services including primary and specialty care, preventive health screenings, immunizations, mental health counseling, pharmacy services and more to patients in Kitsap, mason, and rural Pierce County.<\/p>\n<p><em><strong>\u201cA PCHS patient with a positive FIT Kit test needed transportation to and from for their colonoscopy procedure. PCHS&#8217;s CHW not only provided this patient a ride but also stayed throughout their colonoscopy procedure.\u201d<\/strong> (Health System Annual Reporting 2023)<\/em><\/p>\n<p><strong>Takeaway:<\/strong> Through providing patient navigation services such as transportation assistance, PCHS&#8217;s CHW was able to increase access to CRC screening for its patient and increase access to colorectal cancer screening services.<\/p>\n<p><em>For more information <a href=\"https:\/\/depts.washington.edu\/hprc\/research-impact\/thematic-networks\/arc-nw\/ebi-compendium\/#contact\">contact us<\/a><\/em><\/p>\n<\/div>\n<\/div>\n<\/div>\n<h2 class=\"wp-block-heading has-text-color\" style=\"color:#005e78\">Resources to Support Clinics in Delivering Patient Navigator Services<\/h2>\n<p>Provided below are resources health systems can use to support them in effectively implementing Patient Navigation:&nbsp;<\/p>\n<div id=\"uw-accordion-shortcode\">\n<div class=\"js-accordion\" data-accordion-prefix-classes=\"uw-accordion-shortcode\">\n<div class=\"js-accordion__panel\" >\n<h2 class=\"js-accordion__header\">Patient Navigation Toolkit and Interactive Website<\/h2>\n<p>\n<strong>Source:<\/strong> American Cancer Society<\/p>\n<p><strong>Description:<\/strong> This toolkit is formatted as an interactive website that offers practical advice on paying for and sustaining CRC cancer screening patient navigation to help health care professionals sustain navigation programs.
