Access Cards and Photo ID

HS Building Management programs access cards and prints photo IDs for faculty,  staff,  students, and visitors for the Health Sciences Building.

  • HS Building Management updates access cards when personnel transfer departments, leave the UW or lose their access cards.
  • Only one access card may be active for an individual at a time.
  • Access cards are University of Washington property and must be returned to the issuing department upon termination of appointment, employment, or registration.

With a request form signed by a BAC, individuals may drop in at any time during business hours to obtain a photo ID.  For groups of 3 or more, we kindly request that BACs make an appointment in advance.

Request Form 20

New Cards
  • Fill out the Health Sciences Photo ID and Access Card Request Form 20 (pdf updated 10/2015).
  • The Building Access Coordinator (BAC) should sign the form and provide a budget number.
  • If a HS photo ID must be made, the individual must bring Form 20 to T283 during business hours, as well as photo ID for verification of identity.
  • If this is a new card and a photo is already on file, it can be transferred to the new card. The individual does not need to have the photo taken again, unless desired by the individual.
  • If the person does not need a HS photo, the BAC may mail or fax Form 20 to: Health Sciences Building Management Box 357175 Fax: 616-3116
  • If the individual does not come in for a photo, the BAC will be notified when the card is ready for pick-up in Room T283. (Access cards are not mailed.)
  • If the individual takes their access card, verification will be sent to the BAC of the assignment of a new card and its number.
Reprogram/Reissue
  • To reprogram/re-issue a card, fill out the Health Sciences Photo ID and Access Card Request Form 20 (pdf)
  • Please indicate on the Form 20 the card number you want re-assigned.
  • Have the BAC sign the form and provide a budget number.
  • The individual must bring Form 20 to T283 during business hours, as well as a photo ID for verification of identity.
  • If the person does not need a HS photo ID, the BAC may mail or fax the Form 20 to:

Deactivation

When a cardholder’s status changes, (terminates, moves off-site, transfers to a new department, etc.),  please report this information to Building Management for deactivation of his/her card.

  • Email the cardholder’s name and card number to HS Building Management.
  • Deactivated cards should be kept by the BAC for reactivation.
  • There is no charge for deactivations.
Cost

 

  • Departments will be charged $5.00 for all new and replacement HS photo ID badges.
  • New access cards are $15. Reissued cards are $10.
Requirement
UW Propery

 

  • Photo identification badges are University of Washington property and must be returned to the issuing department upon termination of appointment, employment and/or registration.
  • Each department is responsible for returning badges to Building Management when faculty/staff/students terminate.
