Site Onboarding Tasks
Students in SURG 667 will rotate at multiple facilities, each with varying credentialing requirements and timelines. No less than six weeks prior to the start of the rotation, complete the required compliance paperwork for Providence and Deaconess. Once students have submitted this paperwork, the UW Regional Office will submit the paperwork to the necessary hospitals on your behalf. Once the hospitals process the paperwork, students should receive an email from the hospital that their processing has been completed. Students will need to arrange a time to pick up their badge from the hospitals.

No less than two weeks prior to the start of the rotation, students must contact the site coordinator to confirm participation in SURG 667. Students will receive additional information from the site coordinator approximately one week prior to the start of the rotation with scheduling information and first day activities.
