BILIARY TRACT
Case #1 - A 40 year old lady, with R-sided abdominal pain |
DISCUSSION QUESTIONS: |
7.) How would you proceed with the diagnostic workup?
|
Diagnostic evaluation:
A. Imaging studies: Test of choice is ultrasound examination to see stones, gallbladder wall, size of common duct and hepatic biliary radicles
-Other tests: Plain Xray: only 15% of stones radioopaque so yield is poor
-CT scan: evaluate gallbladder wall and ducts - not indicated here
-Oral cholecystogram: requires time and patient compliance; not possible if bilirubin elevated; 95% accurate in detecting stones.
-HIDA radionuclide scan: normal in biliary colic
B. Laboratory studies: WBC, amylase, bilirubin
