Case #2 -
This 62 year old male underwent emergency open cholecystectomy

DISCUSSION QUESTIONS:
9.) How could postoperative pneumonia be prevented?

Prevention of postoperative pneumonia:

  1. Preoperative instructions regarding deep breathing and ambulation

  2. Smoking cessation at least 2 weeks prior to elective surgery

  3. Planning incisions if possible that are less painful

  4. Proper pain management postoperatively - PCA, epidural catheter

  5. Incentive spirometry and possible intermittent positive pressure breathing

  6. Early ambulation

  7. Avoid fluid overload if at all possible

  8. Keeping the head of the bed at 30 degrees

  9. Avoid unnecessary blood transfusions (ie, using 7.0 as the hemoglobin trigger for transfusion in the stable, resuscitated patient)

  10. Tight blood sugar control in diabetics
