A 30 year old woman with palpitations and weight loss

DISCUSSION QUESTIONS:
4.) What diagnostic workup should you do?

Diagnostic evaluation:


  • Laboratory:


    • TSH - suppressed

    • Free T4 - elevated

    • Thyroid stimulating immunoglobulin - stimulates the TSH receptor and is present in Graves' disease



  • Radioactive iodine scan - possible findings:


    • Increased diffuse uptake in Graves' disease

    • Multiple nodules with varying uptake in Plummers disease

    • Solitary hot nodule with remainder of gland suppressed

    • Thyroiditis with decreased uptake
