A 28 year old man sustained a gunshot wound

8.) How should this patient be managed?

If the patient has increased intraabdominal pressure as well as falling urine output, pO2, and oxygen saturation, the abdomen should be decompressed by opening the abdomen. This is done by opening any existing incision or making an incision if none is present. Decompression is done after adequate volume expansion. There should be immediate clinical improvement. Attempts may be made every 48-72 hours to close the abdomen. If still not possible after 2 or 3 attempts, an absorbable mesh is placed and wound grafted. The resulting hernia is repaired after several months.
