{"id":1038,"date":"2018-05-14T18:30:22","date_gmt":"2018-05-14T18:30:22","guid":{"rendered":"http:\/\/depts.washington.edu\/uwmedptn\/?page_id=1038"},"modified":"2020-10-08T00:14:06","modified_gmt":"2020-10-08T00:14:06","slug":"uw-medicine-empanelment-dashboard","status":"publish","type":"page","link":"https:\/\/depts.washington.edu\/uwmedptn\/strategies-programs\/population-health\/empanelment\/uw-medicine-empanelment-dashboard\/","title":{"rendered":"UW Medicine Empanelment Dashboard"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p>As UW Medicine shifts from \u2018sickness\u2019 to \u2018wellness\u2019 care, the purpose of empanelment is to assign Primary Care Providers and Primary Care teams accountability for a key number of UW Medicine lives.\u00a0 It is therefore important for patients to select and be assigned to a Primary Care Provider (PCP) and Primary Care clinic. Establishing care with a single, UW Medicine PCP ensures every patients&#8217; health care needs are actively monitored in a way that considers patterns of care, and most importantly, the patients&#8217; experiences.
