{"id":1207,"date":"2018-07-30T22:55:55","date_gmt":"2018-07-30T22:55:55","guid":{"rendered":"http:\/\/depts.washington.edu\/uwmedptn\/?page_id=1207"},"modified":"2018-07-31T00:41:06","modified_gmt":"2018-07-31T00:41:06","slug":"lung-cancer-screening-path","status":"publish","type":"page","link":"https:\/\/depts.washington.edu\/uwmedptn\/strategies-programs\/population-health\/primary-pathways-path-initiatives\/path\/lung-cancer-screening-path\/","title":{"rendered":"Lung Cancer Screening PATH"},"content":{"rendered":"<p>To meet our mission of providing broad access to the best care, and with empaneled patient data now available, the PATH (population approach to health) team has started a system-wide intervention to increase rates of early-detection of lung cancer through evidence-based screening. Doing so will close a gap in the early-detection of lung cancer among Medicare, Medicare Advantage, Medicaid, commercially insured individuals (including ACN and value-based contracts), and eligible individuals paying out-of-pocket. In addition to achieving our mission goals, this intervention will put us \u201cahead of the curve\u201d in meeting anticipated quality measures that payers will use to pay for performance.<br \/>\n<strong><br \/>\nTarget Patient Population<\/strong>: This includes current or former smokers (and those with missing smoking status) aged 55-80 years, who receive care at HH, HMC, NWH, UWMC, and UWNC.<\/p>\n<p><strong>Goals:<\/strong><br \/>\n1) more complete and accurate smoking status information on UW Medicine Primary care empaneled patients;<br \/>\n2) more complete and accurate lung cancer risk factor documentation within the EHR for UW Medicine Primary care empaneled patients who are current or former smokers;<br \/>\n3) higher rates of shared-decision making visits and lung cancer screening among high-risk individuals;<br \/>\n4) a dashboard for monitoring provider, clinic, and system-level performance.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-1208 size-full-content\" src=\"http:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer-750x563.jpg\" alt=\"\" width=\"750\" height=\"563\" srcset=\"https:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer-750x563.jpg 750w, https:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer-300x225.jpg 300w, https:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer-375x281.jpg 375w, https:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer-108x81.jpg 108w, https:\/\/depts.washington.edu\/uwmedptn\/wp-content\/uploads\/lung-cancer.jpg 960w\" sizes=\"(max-width: 750px) 100vw, 750px\" \/><\/p>\n","protected":false},"excerpt":{"rendered":"<p>To meet our mission of providing broad access to the best care, and with empaneled patient data now available, the PATH (population approach to health) team has started a system-wide intervention to increase rates of early-detection of lung cancer through evidence-based screening.
