{"id":622,"date":"2018-03-15T23:44:11","date_gmt":"2018-03-15T23:44:11","guid":{"rendered":"http:\/\/depts.washington.edu\/uwmedptn\/?page_id=622"},"modified":"2018-06-22T18:22:15","modified_gmt":"2018-06-22T18:22:15","slug":"primary-pathways-path-initiatives","status":"publish","type":"page","link":"https:\/\/depts.washington.edu\/uwmedptn\/strategies-programs\/population-health\/primary-pathways-path-initiatives\/","title":{"rendered":"Primary Care Pathways\/PATH Initiatives"},"content":{"rendered":"<h4>About Primary Care Pathways (Pathways):<\/h4>\n<p>UW Medicine is developing ambulatory clinical care pathways which will allow for a system-wide, evidence-based approach to delivering predictably excellent care to patients in primary care.<\/p>\n<p><strong>Goals:<\/strong><\/p>\n<ul>\n<li>Reduce unnecessary variation in patient care by building consensus around evidence based protocols and reducing unnecessary procedures, interventions, tests, or labs.<\/li>\n<li>Optimize the health and experience of patients with certain conditions and diseases. \u00a0Ambulatory pathways are a standard tool to help providers, nurses and other clinical team members to implement comprehensive, individualized care plans for their patients using the most up-to-date information and connections to resources in the community.<\/li>\n<li>Improve care coordination for chronic disease management.<\/li>\n<li>Support the use of evidence based guidelines in key areas of opportunity in clinical practice such as, depression, hypertension, diabetes and low back pain.<\/li>\n<\/ul>\n<p>Find out more <a href=\"http:\/\/depts.washington.edu\/uwmedptn\/?page_id=710&amp;preview=true\">here<\/a>.<\/p>\n<h4>About Population Approach to Health (PATH):<\/h4>\n<p>PATH is an outlier management program designed to track and monitor sub-populations of patients who have \u201cfallen off the Pathway\u201d.
