Neurology Patient Encounter Log

Instructions

• Keep track of the patients you are seeing in the pocket syllabus.  Please record your patient encounters in E*Value by the end of the 3rd week.  https://sites.uw.edu/medevalu/clinical-encounters/
• Seven different disease categories are required.  (See table below)  Only one patient in each category is needed. 
• You may record two diagnoses for one patient if both are significantly discussed.
• In E*Value there are 3 options to record your involvement.  Participation is the goal.  The alternative method (online case) does meet the requirement, but should be recorded in the last week of the clerkship only if participation does not happen.  Observation does not meet the requirement for neurology. 
• The degree of involvement to meet the definition of "participation" is face-to-face with transfer of information.  The goal is that you participate in their care by taking the history, performing the physical exam, delivering the oral presentation and/or writing a note.

Examples that meet the participation criteria:

1. You evaluate a patient in the ER, clinic or in the hospital on your own.
2. You share the evaluation with another practitioner (MD, DO, PA, ARNP), but you also participate to some degree with the Hx, PE, orders or notes.

Examples that don't meet the criteria:

1. Discussion of a patient who is not physically present.
2. Follow-up visits in clinic or on the wards in which only a small part of the problem is discussed such as response to an antiepileptic or headache medication.
3. Shadowing your preceptor with no participation.
   
   
• We will monitor the log. Based on your responses, you may receive an email at the beginning of the 4th week reminding you to complete the log.  Example email:  CEREBROVASCULAR DISEASE (Ex: TIA, ischemic, hemorrhagic):  Add this if you forgot OR talk to your preceptor about seeing a patient OR fill in the log for alternative method because you already did/will do as make-up case 2, 3 or 4.
