Instructions for completing the reporting survey

You will be using this survey to report the activities and impacts associated with your BTOP grant. You must submit this survey to receive reimbursement for your eligible expenses. The survey due dates are as follows:

    • Fifth Report (January 1, 2012 – March 31, 2012) due April 8, 2012
    • Sixth Report (April 1, 2012 – June 30, 2012) due July 8, 2012
    • Seventh Report (July 1, 2012 – September 30, 2012) due October 8, 2012
    • Eighth Report (October 1, 2012 – December 31, 2012) due January 8, 2013

Evaluation Packet

We have created a spreadsheet to help you track your training activities on a daily basis, and a sample client survey and survey tracker. These materials are designed to work with the online survey, but you may find it necessary to customize them based on your center type. Please feel free to email if you wish to discuss additional strategies for collecting impact data about your clients.
