Diagnosis-Specific

Advocacy groups and experts have developed tools and materials on transition and health that specifically address several diagnoses. If your diagnosis of interest is not included below, check the websites of advocacy groups for the diagnosis. You can find these by looking the diagnosis up on NORD or other similar websites. Autism

  • Autism Speaks Transition Toolkit

    A booklet containing information and resources for adolescents living with autism to use in planning for transition to adult life. Includes sections on Self-Advocacy, the Transition Plan, Community Living, Employment, Post-Secondary Education, Housing, Health, Legal, Internet and Technology, and Getting Organized. Each section has resources. Can be downloaded free of charge from Autism Speaks website after registering on the website. Families of adolescents and young adults with autism between the ages of 14 and 22 may request a complimentary hard copy of the Transition Tool Kit by filling out the Transition Tool Kit order form or calling 888-AUTISM2 (888-288-4762) and speaking with an Autism Response Team Coordinator.
