THORACIC SURGERY
Case #3 -
A 62-year-old man complains of worsening cough since three months

DISCUSSION QUESTIONS:
11.) What is the management of the primary if the metastatic workup were negative?

Primary management for non-small cell lung cancer:

  1. Early stage (Stage I or II): surgical resection by lobectomy or pneumonectomy. Controversy exists regarding adjuvant therapy for Stage II NSCLC

  2. Stage III (locally advanced disease with nodes):

  3.         Surgery may not be feasible

            Radiotherapy alone gives poor results

            Chemoradiation gives better results

Neoadjuvant chemotherapy followed by surgery may be best
  4. Stage IV: chemotherapy
