A 40 year old man with reflux symptoms

DISCUSSION QUESTIONS:
5.) What is an appropriate diagnostic workup in this patient? What do you hope to learn with each study?

Appropriate diagnostic evaluation:



  • Barium swallow / upper GI series:

    Esophageal motility, stricture, status of gastric emptying

  • Esophagogastroduodenoscopy:

    Presence and length of esophagitis; detection of carcinoma or Barrett's esophagus; detection of peptic ulcer disease

  • Esophageal manometry:

    Evaluate lower esophageal sphincter (normal pressure >6mm Hg)

  • 24 hour pH monitoring:

    Not needed if typical symptoms and erosive esophagitis at endoscopy
