Case #5 - A 90 year old with abdominal pain and vomiting

 
CLINICAL CASE:
You are asked by the medicine service to evaluate worsening abdominal pain, nausea, and vomiting over the last 3 days in a 90 year old thin lady. The abdominal films are consistent with a small bowel obstruction. On physical exam her abdomen is distended, tender and she has a 5 cm non-reducible lump in her right groin.
