A 55 year old with acute R leg and foot pain

DISCUSSION QUESTIONS:
8.) What do you do preoperatively about his anticoagulant therapy? What do you do postoperatively?

Prior to surgery, the INR should be checked. If therapeutic or higher, administration of fresh frozen plasma will probably be necessary before an arteriogram or surgery. After surgery the patient will need heparin and then long term Warfarin to prevent further embolic events.
