9.) What are indications for operative management?

Operative management is indicated if the pneumatic reduction is unsuccessful, if nonviable bowel is suspected, if an anatomical lead point is suspected, or if perforation complicates an attempted pneumatic reduction.
At surgery the intussusception may be reduced. If that is unsuccessful, if the bowel is not viable, or a lead point is identified, then resection is indicated. Otherwise reduction and appendectomy complete the operative procedure.
