4.) What is the first diagnostic test that should be done and why?

The first study that should be done is the barium swallow. This will not only show the level of obstruction and the possible cause but also will give information about esophageal motility. Achalasia results in a typical bird-beak deformity in the distal esophagus with dilatation of the proximal esophagus and diminished or absent peristalsis in the body of the esophagus.
