Unusual Presentation of Clostridium difficile Colitis

A gentleman in early 70s s/p porcine mitral valve replacement a year prior presented to Emergency Department with 4 hour history of fever & rigors, suspecting for endocarditis. Patient had otherwise been well. He had had a colonoscopy approx. 2 weeks prior.

Later during his hospitalization, he described loose stools occurring 2-3x/day every day since his colonoscopy.

Toxigenic C. difficile was detected by PCR in his stool. His symptoms resolved after 4 days of antibiotics treatment.
