Case 7
Diagnosis: ALS
The keys to this case are:
- Motor only which generally puts you in the categories of motor neuron, neuromuscular junction and muscle.
- Gradually progressive proximal and distal disease without fatigability.
- Asymmetric bilateral without eye involvement.
- Recognizing lower motor neuron signs including fasciculations, weakness, atrophy, and the EMG test.
- Recognizing upper motor neuron signs including weakness and retained reflexes in the face of 3/5 weakness. If all of the disease was LMN, the reflexes would be absent.

Diagnosis: MIGRAINE HEADACHE
The keys to this case are:
- Nausea, photophobia, sonophobia, and vomiting are all common symptoms of migraine.
- Migraine duration is usually a few hours to days.
- Not responding to anti-migraine drugs does not rule out migraine.
