Case 13a

Diagnosis: PARKINSON DISEASE

The keys to this case are:

  1. The 4 cardinal features of PD are present including resting tremor, bradykinesia, gait instability, and rigidity.
  2. She is not on a drug that could cause a parkinsonian side effect.

Case 13b

Diagnosis: PARKINSON PLUS SYNDROME (PROGRESSIVE SUPRANUCLEAR PALSY)

The keys to this case are:

  1. Prominent gait trouble and rigidity without resting tremor.
  2. No response to Sinemet.
  3. Limited eye movements (downgaze).
  4. Recognize that there are lots of diseases under the heading of parkinsonism that aren’t Parkinson disease.

Case 15

Diagnosis: BENIGN POSITIONAL VERTIGO

The keys to this case are:

  1. Vertigo that is described as positional lasts < 2 min.
  2. There are no central features making this likely a peripheral vestibular process.
  3. Vertigo and nystagmus were produced by the Hallpike maneuver.
