4.) In addition to carotid disease, what else could cause TIA's like this patient had?

Transient ischemic attacks may be caused by carotid disease, cardiac disease or aortic arch atherosclerosis giving rise to embolus, or intracranial disease. Rare causes include brain tumor, aneurysm, and arteriovenous malformation.
