Students in SURG 611 will rotate at multiple facilities, each with varying credentialing requirements and timelines. No less than six weeks prior to the start of the rotation, complete the required compliance paperwork for Providence and Multicare. Once students have submitted this paperwork, the UW Regional Office will submit the paperwork to the necessary hospitals on your behalf. Once the hospitals process the paperwork, students should receive an email from the hospital that their processing has been completed. Students will need to arrange a time to pick up their badge from the hospitals.

If you have hospital credentialing questions or issues accessing onboarding paperwork, please reach out to Kelly Juarez (kellyj13@uw.edu) in the UW Regional Office.

Summit Cancer Center has multiple locations across the Spokane area, as well as a location in Couer D’Alene. Students participating in this rotation could rotate through different clinics and hospitals from day to day based on which site has the best educational experience and case type. Dr. Amsbury will work with students to identify which location to report to. Given the various locations that the physicians practice and operate at, it is critical for students to have reliable transportation.
