  • With this application, you can either register for a single course (Course 2: Operational Quality Management in the Clinical Laboratory) at the single course rate, or you can register for the full program (which includes 3 courses).
  • If applying for the full program (3 courses) at this time, you are eligible for a discount. Note: This program discount is only available if all 3 courses are purchased at the time of application.
  • Please note that for the full program, the order of the courses you will be enrolled in is listed below.
  • If you previously enrolled in Course 1 or Course 3 at the single course rate and would now like to take Course 2, please select the "single course" option below.

Courses by Date Offered:

  • Course 2: Operational Quality Management in the Clinical Laboratory (January 21 – March 30, 2025)
  • Course 3: Quality Control Methods in the Clinical Laboratory (April 7 – June 15, 2025)
  • Course 1: Leadership and Management in the Clinical Laboratory (September 29 – December 7, 2025)

Cost:

  • Single course: $200 USD (LMIC rate) / $500 USD (OECD rate)
  • All three courses (entire certificate program): $500 USD (LMIC rate) / $1200 USD (OECD rate)

Policy

Course Participation: Please plan to spend 6-9 hours per week reviewing course materials and completing assignments.
Refunds: Full refunds will be issued for a course if we are notified up to one working day prior to the start of the course. If you paid for the complete course program at the discounted rate and are unable to proceed with one of the courses in the program, you may contact us about the possibility of a credit for that course so you can complete the program at a later date. Please contact us prior to the start of the course.
Certificates: Upon successful completion of the course, we will issue you a certificate of completion based on the name provided in the personal information section. Changes to certificates after the course ends will be subject to a $10 replacement fee. More information about certificates.
Certificate Program Communications: After applying for a course, we may communicate with you about upcoming courses in Laboratory Medicine and application dates. If you do not wish to receive these communications, you are welcome to opt-out or you can contact dlmp-cert@uw.edu to request this.

Course Policy Acceptance

Payment is required in order for your application to be complete and is due immediately, or by 7 January 2025 (when applications close) at the very latest.
