Instructions

• Verify that you comply with the eligibility
• Download the PREP information flyer which provides instructions on the required application materials
• Download, complete, and send the PREP-Application. Supply only the last 4 digits of your SSN
• Submit a copy of your ID
• Submit a statement of interest
• Submit three letters of recommendation
• Submit a copy of unofficial transcripts from all colleges and universities you attended. Official transcripts are required upon acceptance and previous to the start of the program
• Submit a CV/resume

Submit all materials in PDF form as attachments to a single email to uwprep@uw.edu by February 1, 2025.
