girl on laptopThere are many different types of resources about health and health transition available to teens, families and professionals on the internet. We have provided resources in three different categories: Tools, Websites, and Support Groups.

In the Tools section, you can find direct links to forms, checklists, modules and booklets. In the Websites section, there are links to websites that focus on health, transition or health transition. Finally, in the Support and Advocacy Groups section there are links to groups for teens, young adults and families that promote self-advocacy, disability policy advocacy, or provide support.
When you turn to the internet for information, it is a good idea to understand how to promote safety and avoid unreliable information. The following links can help you with these issues:
