How to Order Instruments:

  1. As of July 1, 2019, all instruments (except for the WHOQOL-BREF) will be priced as follows: free for students, $300 for academic/non-profit organizations, and $900 for private/for-profit institutions.
  2. Please submit your requests to the Instrument Distribution Coordinator by filling out the:   Instrument Order Form and User Agreement
    (Please make sure to give complete information as to the purpose of your study, and what you are requesting.)
  3. If you have and questions about the Instrument Order Form and User Agreement please contact the Instrument Distribution Coordinator:

  4. When your submitted Instrument Order Form and User Agreement is processed, the Instrument Distribution Coordinator will send you an email with an invoice. Use this to verify your requests and the amount due.
  5. The requested instrument/translations will not be sent until payment or a purchase order is received.
  6. Instrument documents will be emailed to you as electronic files.
  7. DO NOT TRANSLATE ANY INSTRUMENT OBTAINED FROM SEAQOL WITHOUT WRITTEN PERMISSION. PLEASE CONTACT US FOR INSTRUCTIONS ON TRANSLATIONS AND CULTURAL ADAPTATIONS.

Description of Instrument Charges

The SeaQoL instruments are widely available to users. These charges cover our costs only, we do not market our instruments on a for-profit basis. There is NO per-subject fee.

The cost covers:
  • the writing and updating of user manuals, including scoring programs and sample datasets;
  • maintenance of our website; and
  • the administration, preparation and distribution of related instrument documents.

For orders within the United States:
  • We have different pricing for students, academic researchers/non-profits/public institutions and for-profit users.
  • Payment for domestic orders must be made by check, money order or purchase order before receipt of instruments. We cannot accept credit cards.

For International orders:
  • International orders must be paid by USD bank checks/money order (payable through a US bank) or wire transfer.
