The Incubation Period of Severe Acute Respiratory Syndrome Coronavirus 2A Systematic Review

[pre-print, not peer reviewed] A systematic review of 64 articles aggregating the experiences of 854 patients suggests an estimated median incubation period (time from exposure to symptom onset) of 4.9 days (95% CI 4.6-5.2). However, including 81 asymptomatic and presymptomatic patients, along with 31 outlier cases with incubation periods exceeding 14 days, Li et al. estimate that people in the 97.5 percentile would…

Reprocessing Filtering Facepiece Respirators in Primary Care Using Medical Autoclave: Prospective, Bench-to-Bedside, Single-Centre Study

Thirty-three respirators were decontaminated with a medical autoclave up to three times and tested for loss of structural integrity and ability to filter small particles. FFP-2 respirators performed well: all the samples retained their shape and at least 90% of their filtering capacity after one, two, and three decontaminations, suggesting that they might be successfully…

Characteristics of 24516 Patients Diagnosed with COVID-19 Illness in a National Clinical Research Network Results from PCORnet

[pre-print, not peer reviewed] An analysis of data from 36 large healthcare systems in 29 U.S. states found a total of 24,516 patients across healthcare settings were diagnosedwith COVID-19 between January 1, 2020, and May 18, 2020. In the same time period, 79,639 were diagnosed with viral pneumonia and 163,984 were diagnosed with influenza.   Comorbidities were common among COVID-19 patients,…

The Risk and Prognosis of COVID-19 Infection in Cancer Patients: A Systematic Review and Meta-Analysis

A systematic review and meta-analysis (n=22 studies with 1,018 cancer patients) found the prevalence of cancer among patients was 2.1% (95% CI 1.3, 3%). Case fatality among these patients was 21.1% (95% CI 14.7, 27.6%). The probability for severe/critical disease was 45.4% (95% CI 37.4, 53.3%), for ICU admission was 14.5% (95% CI 8.5, 20.4%), and need for mechanical ventilation was 11.7% (95% CI 5.5,…

Symptom Relief Is Possible in Elderly Dying COVID-19 Patients: A National Register Study

Strang et al. used the Swedish Register of Palliative Care, which has national coverage, to compare symptoms and symptom control in the last week of life among patients dying of COVID-19 in nursing homes and hospitals (n=390) to registered deaths in similar settings in 2019 (n=46,698). They found breathlessness was more common in COVID-19 patients than the reference population, and in hospitals than in nursing homes. Relief of breathlessness,…

BMI Is Associated with Coronavirus Disease 2019 Intensive Care Unit Admission in African Americans

A cross-sectional study of African American patients with COVID-19 treated in a single tertiary care center (n=158) between March 12 and April 9 found older age (OR 1.07, 95% CI 1.03, 1.11), higher BMI (OR 1.12, 95% CI 1.05, 1.18) and lung disease (OR 3.1, 95% CI 1.4, 8.4) were significant risk factors for ICU admission. In contrast…

SARS-CoV-2 Seroprevalence Across a Diverse Cohort of Healthcare Workers

[pre-print, not peer-reviewed] A study among workers at a multi-site healthcare system in LA County estimated SARS-CoV-2 antibody seroprevalence post–stratified on demographic group (n=15,000 invited; 6,062 participated). Overall seroprevalence was 4.1% (95% CI 3.1, 5.7%), with lower estimates in older workers (aOR = 0.8, 95% CI 0.7, 0.9) and higher estimates in Hispanic workers (5.7%; 95% CI 3.9, 8.3%; aOR = 1.8, 95% CI 1.3, 2.4). Significantly higher estimates were also observed for African American race (aOR=1.8, 95% CI 1.1, 2.9), and for workers…

Vertical Transmission of COVID-19: A Systematic Review and Meta-Analysis

Kotlyar et al. pooled results of from 38 studies with 936 neonates to investigate SARS-CoV-2 vertical transmission. They found that the proportion of vertical transmission based on RNA PCR results from infant nasopharyngeal swabs was 3.2% (95%CI 2.2-4.3%). The neonatal serology (IgM) was positive in 3.7% (3/82). The authors note that the majority of mothers in their sample were likely infected during the third trimester of pregnancy, and that the risk of vertical transmission as…

Decontamination and Reuse of Surgical Masks and N95 Filtering Facepiece Respirators during COVID-19 Pandemic: A Systematic Review

Seresirikachorn et al. evaluated the effectiveness of existing decontamination methods of surgical masks and N95 respirators with a systematic review of 15 studies published before April 11, 2020 that described 14 decontamination methods. They find a low level of evidence supporting the use of four of the methods for decontamination and reuse of N95 masks, including ultraviolet germicidal irradiation, moist heat,…

Post‐discharge Symptoms and Rehabilitation Needs in Survivors of COVID‐19 Infection: A Cross‐sectional Evaluation

Halpin et al. evaluated 100 COVID-19 survivors between 29 and 71 days after hospital discharge, including 32 people who had required ICU admission. Commonly reported symptoms among recovered patients included new illness-related fatigue (72% of the ICU group and 60% of the general admission group), breathlessness (66% of ICU group, 43% of general admission), and psychological distress (47% of ICU group, 24% of general admission).   Survivors were also…
