This “care transformation” advances patient-centered care, with a focus on achieving the “Quadruple Aim” – improved patient care, better health and outcomes for patient populations, better value and clinician satisfaction.
UW Medicine received a Transforming Clinical Practices Initiative (TCPI) award from Medicare and Medicaid Innovations (CMMI) in October 2015 to support care transformation efforts in Washington, Wyoming, Alaska, Montana and Idaho (WWAMI).
