Accounting of Disclosures
Patients have the right to receive an accounting of certain disclosures of their protected health information for a time period of up to 6 years prior to the date on which the accounting is requested. All disclosures that are not specifically excluded from the accounting must be provided in the written accounting.
UW Medicine Compliance is responsible for receiving and processing requests for accounting of disclosures.
Requests for an accounting of disclosures are made to UW Medicine Compliance who compiles a report, meeting all regulatory requirements, from all UW Medicine entities as applicable. All disclosures that are not executed and documented by the entities’ Health Information Management service areas are documented in the Accounting of Disclosures database by the workforce members executing those disclosures.
The database developed to track disclosures from other sources, such as research or mandatory reporting, supports either single entry submission via a web form or batch data entry via a spreadsheet. How to Report Accounting of Disclosures.
