Accounting of Disclosures
Individuals have a right to receive an accounting of certain disclosures of Protected Health Information [PHI] for a time period of up to 6 years prior to the date on which the accounting is requested. All disclosures that are not specifically excluded from the accounting must be provided in the written accounting.
UW Medicine Privacy Officials are responsible for receiving and processing requests for accounting of disclosures.
Requests for an accounting of disclosures are made through each UW Medicine entity’s Privacy Official. The Privacy Official gather reports from all the entities and integrates the materials into a report for the requestor.
This written accounting includes the date of disclosure, name and address of recipient, description of information disclosed, and the purpose of the disclosure or such other request for disclosure.
All disclosures that are not executed and documented by the entities’ Health Information Management Service Areas are documented in the Accounting of Disclosures database by the workforce members executing those disclosures.
The database developed to track disclosures from other sources, such as research or mandatory reporting, supports either single entry submission via a web form or batch data entry via a spreadsheet. How to Report Accounting of Disclosures.
