Support and Alignment Networks

SANs

Support and Alignment Networks (SANs) are funded by the Transforming Clinical Practices Initiative (TCPI) to provide support and resources to practice transformation networks (PTNs), peer-based learning networks designed to coach, mentor and help clinicians develop core skills to transform care.

SANs provide a system for workforce development using professional associations and public-private partnerships currently working in care transformation endeavors. They ensure sustainability of these efforts and especially support recruitment of practices serving small, rural and medically underserved communities.

SANs:

  • Help recruit clinicians to PTNs
  • Provide continuing medical education
  • Support practices with patient and family engagement

SANs offer a variety of resources for clinical quality improvement and practice management at no cost to PTN-enrolled practices and providers. A summary of these offerings is provided below. American Board of Family Medicine (ABFM)

ABFM engages primary care practices. They offer access to the PRIME Registry to support practice quality measure extraction, provide feedback to clinicians and practices, monitor outcomes and report to the Physician Quality Reporting System (PQRS) and Meaningful Use.

American College of Emergency Physicians (ACEP)

ACEP engages emergency department (ED) clinicians. They offer learning events and quality improvement resources in geriatric ED medicine, improving sepsis outcomes, reducing ED admissions for chest pain and reducing avoidable imaging and testing in the emergency department.

American College of Physicians (ACP)

ACP engages all practices. They offer access to the ACP Practice Advisor, a web-based practice improvement tool with more than 40 modules in these key areas:

  • Building the patient-centered medical home
  • Improving clinical care for populations with chronic conditions
  • Managing practices
  • Specialty practice patient-centered care

American College of Radiology (ACR) R-SCAN Network

R-SCAN engages radiology and referring practices. It offers access to a clinical decision support tool to guide quality improvement projects. It also offers a step-by-step plan for engaging referring clinicians to improve the ordering of appropriate imaging exams based on 10 Choosing Wisely guidelines.

American Medical Association (AMA)

The AMA engages all practices. They offer STEPS Forward, a web-based practice improvement tool with more than 25 modules in key conceptual areas in practice transformation. It also offers learning events and quality improvement resources in improving blood pressure control and preventing Type 2 diabetes.

American Psychiatric Association (APA)

The APA engages psychiatrists. They offer online and in-person training in the collaborative care model in which primary care providers and behavioral health specialists work together to treat behavioral health patients.

Care Coordination and Transition Management (CCTM)

CCTM engages nurses in primary care practices. UW Medicine is offering PTN enrollees access to the American Academy of Ambulatory Care Nursing’s CCTM course. These online courses cover nine dimensions of the registered nurse’s (RN) CCTM role and related competencies for care coordination and transition management.

National Nursing Centers Consortium (NNCC)

NNCC engages nurse practitioners in all practices. Nurse practitioners receive free technical assistance and peer mentoring to improve their quality improvement capacity, achieve practice transformation, and transition to quality-based payment structures. NNCC also offers toolkits to support business operations of nurse-managed practices.

Network for Regional Health Improvement (NRHI)

NRHI engages all practices. It brings together over 30 regional organizations that collaborate on joint projects in cost, quality and population health with dissemination of best practices within each respective local region. It also offers learning events and quality improvement resources for various high-value care topics, including understanding total cost of care and improving patient and family engagement.

Patient-Centered Primary Care Collaborative (PCPCC)

PCPCC engages primary care practices. They provide assistance with engaging patients and family caregivers with the care team, including training and resources for clinicians and for patients and caregivers partnering with the practices.

PCPCC also offers workshops on how to improve patient and family-centered care and assessing patient experiences. Additionally, PCPCC connects practices to community health organizations to build partnerships in population health and wellness.
